Health Care Decisions

All adults can benefit from thinking about what their healthcare choices would be if they became unable to speak for themselves. We encourage everyone to think about their options, discuss their wishes and concerns with family, friends and healthcare providers, and put this information in writing. Through our website, we can provide tools and resources to assist you with this important task. 

Advance Directives

 

There are two types of advance directives: A Living Will and a Health Care Representative.

  A healthcare representative is a person you select to speak on your behalf about healthcare decisions, in the event that you are unable to speak for yourself .
   
  A living will documents what kinds of medical treatments you would or would not want at the end of life.

Aging With Dignity (Five Wishes)
Caring Connections offers free, state-specific advance directives for all 50 states and DC that meet the legal requirements for each state. Download individual copies for free or call 800.658.8898 to have a copy mailed to you.

Center for Practical Bioethics
Caring Conversations is a workbook to help individuals and families communicate with each other about their healthcare preferences and contains advance directive documents.
