Mrs. Carlan comes to Masonicare with several years of healthcare experience, including her years as a registered nurse at Stamford Hospital and most recently, as supervisor of clinical services for VNHC of Southwestern Connecticut.  In her role as clinical supervisor of the hospice program, Mrs. Carlan oversees a host of comprehensive services provided by the hospice team, including pain management and the expert nursing care and support provided to hospice patients in their homes or in area healthcare facilities.

Mrs. Carlan holds a Bachelor of Science Degree in Nursing from Fairfield University and a Bachelor of Science Degree in Psychology from Sacred Heart University.  She also has certification in Hospice and Palliative Care from the National Hospice and Palliative Care Organization (NHPCO).

Mrs. Carlan resides in Norwalk with her husband and three sons, and is active in the community's various youth sports programs.

In addition to providing outstanding nursing care, the Norwalk office's interdisciplinary hospice team includes specially trained hospice volunteers.  Together, they offer a wide range of support and counseling services for patients and families, including grief counseling, spiritual counseling, resource planning and assistance with end-of-life issues.  Complementary therapies such as Reiki, expressive arts activities, music therapy and pet visits help to make the end of life as smooth and comfortable as possible.

Representatives of the Norwalk hospice team are available to speak to community and civic organizations on a variety of topics that can explain how hospice care can help patients and families make the most of their remaining time together.  For additional information about Masonicare's Home Health and Hospice program or to arrange for a speaker for your organization, please call Jennifer Carlan in the Norwalk Office at 203-831-8882.

In addition to Masonicare Home Health & Hospice, which is statewide, Masonicare's affiliates include Masonicare Health Center and Masonicare at Ashlar Village in Wallingford, Masonicare at Newtown, and Masonicare Partners Home and Health and Hospice, a collaboration between Masonicare and St. Francis Hospital and Medical Center in Hartford.
